The Supportive Care Nurse provides one-to-one care at the bedside of terminally ill patients, based on an ongoing nursing assessment of need and in accordance with the established plan of care. Full time employment is three 12 hour shifts per week including weekend rotation, full time position is eligible for $5,000 bonus , see details below. PT and PRN work also available . Duties include:
· Provides compassionate end-of-life care in accordance with the individualized plan of care, with demonstrated ability to assess and respond to patient and family needs in a timely manner
· Educate patients and caregivers on end-of-life, services and other resources
· Document complex medical information using clinical tools, data and other information to complete assessment of patient needs
· Establishes professional and collaborative relationships with patients, families and providers utilizing strong written and verbal communication skills.
· Basic computer skills, including working knowledge of the Microsoft Office suite
· Night shift: 8:00 pm - 8:00 am, three (3) shifts per week including every other weekend
· Care Site - private residences, nursing homes, assisted living facilities, in Kentucky and Ohio
· SIGN ON BONUS! ** Bonus $2,500 after six months of full-time employment, and $2,500 after one year of full-time employment**
Requirements:
PROFESSIONAL: Graduate of an accredited School of Nursing and current licensure, or eligibility to be licensed, in Kentucky and Ohio. At least one year of experience in a patient care position in a hospital, nursing home, or related health care facility preferred.
PERSONAL: Personal qualities of empathy, responsibility and dependability. Must demonstrate effective communication skills with team members, patients, providers, and family members. Must have ability to work independent of direct on-site supervision. Demonstrates leadership and teaching ability.
PHYSICAL JOB: Must arrange travel and be able to access client homes, nursing homes, hospitals, and other facilities independently. Must demonstrate proper use of personal protective equipment and observe universal precautions. Must be able to lift at least 50 pounds using proper body mechanics. Must have valid driver's license, current auto insurance and reliable transportation.
